Apply for A Grant
The following information must be provided to request a Grant from the Oregon Sports Authority Foundation (OSAF). Upon completion of the form below, please send an e-mail to info@oregonsports.org with key organizational documents, such as articles of incorporation, bylaws, charter, board of directors or similar documents, as well as financial reports, including operating and capital budgets, a line item budget for the project you are requesting we fund, including bid estimates from vendors, and other pertinent financial information.

The Oregon Sports Authority Foundtion requires all Grant Recipients to provide OSAF with a final written report of the use of Grants awarded. Please submit your report to the OSAF office at 1888 SW Madison, Portland, OR 97205. If you have questions about this report, please contact us. The report should contain the following:
- Short summary of project objectives and how they were met
- Project results, including number, characteristics and demographics of those served
- A summary of income and expenses for the project funded by Grant
- A breakdown of how OSAF grant was used
- A list of other foundations, organizations and corporations contributing to the project including the $ amount of their support.
- Examples of materials used to promote your organization project and the OSAF
- Examples of media coverage and other exposure resulting directly or indirectly from use of provided toolkit
- Your comments and suggestions for improving the Grant process and materials provided by OSAF
